Collected Personal Data 

• I will collect data (your personal info such as contact details and a consultation form) and keep this for myself and won't share with a 3rd party, unless receptionists at the places I practice at need to know a contact number or email address for safety or emergency reasons.
   
• I will keep this information until we no longer see each other for treatments or until you ask me to delete them.

Purpose of collecting data

• I will keep your data for insurance reasons; a client consultation form acts as a contract between us prior to treatment and also as an information provider should I/receptionist need to contact someone who knows you in case you fall ill or there is an emergency regarding your treatment.
